Andrews was named Parliamentary Secretary for Health in the Steve Bracks Labour government after being elected to parliament in the Legislative Assembly seat of Mulgrave in 2002. He was appointed to the Cabinet following the 2006 election, becoming Minister for Gaming, Minister for Consumer Affairs, and Minister Assisting the Premier on Multicultural Affairs. Andrews was appointed Minister for Health in the John Brumby Labour cabinet in 2007. Andrews voted in favor of abortion law reform in Victoria in 2008. During the passage of the Abortion Law Reform Act in 2008, Andrews sought advice from prominent church clerics, who informed him that the act was antithetical to Church doctrine. Andrews responded that he “…had no intention of becoming a Catholic health minister.” My ambition was to become the Victorian Health Minister.”

Brumby resigned as leader of the Victorian Labour Party after the Labour Party was defeated in the 2010 election, ending 11 years of Labour rule in Victoria. Andrews was elected Victorian Labour Party leader on December 3, 2010, becoming Leader of the Opposition in Victoria, with former Deputy Premier Rob Hulls remaining as his deputy. Hulls retired in early 2012, and James Merlino took over as deputy.
